If you do not have the free software Adobe Acrobat Reader, click here or on the logo and follow the steps. For instructions on downloading, saving and printing these forms, click here.

The fillable forms can be completed, printed and faxed to DCI or completed, saved, attached and emailed to investigation@dci-inc.org

Downloading & Saving Instructions

Downloading Instructions:
Follow the instructions below to download and save these forms to your hard disk.

1. Right click on the link (Mac users: click and hold on the link).
2. Select the Save Target As... option from the pull-down menu that appears (Netscape users: select the Save Link As... option).
3. Choose the destination folder where you want to save the form.
4. Click Save and the form will begin to download to the chosen location.

Note: If prompted to save as format Source or Text, select Source.

Printing Instructions:
Click on the appropriate form above. This will open a PDF file within your web browser. These forms can be printed from your web browser by clicking on the print icon or selecting "Print" from the "File" menu.
